Non-Student Membership

If you are a member of the NC State community, you may be eligible for a Carmichael Complex membership. Please bring your Wolfpack One Card to the Member Services Suite for membership registration.

If you are an NC State alumni, please bring your NC State Alumni Association membership card.

We ask that you provide a letter of employment on official NC State letterhead with empoyment dates from your sponsoring department for visiting staff, faculty and temporary employees.

For your convenience, you may fill out the front of this form prior to your enrollment at the Member Services Suite.

Household Membership

You may sponsor up to three household members, 18-years or older at an additional cost for each membership. We will ask for proof of same address at the time of membership registration and renewal.

Payment Options

Annual

Annual membership is based on a one-year contract from the date of joining. Membership must be paid in full by cash, check, Visa or Mastercard. Cancellation during the one-year contract period will only be permitted in cases of physician-verified illness, a move beyond a 50-mile radius from NC State, or employment termination. A $30 service fee will apply to all cancellations during the one-year contract period.

Month-to-Month

Fees must be paid in full by cash, check, Visa or Mastercard. Multiple months may be purchased at one time.

Payroll Deduction
Faculty/Staff, Faculty/Staff Household Only

This membership is based on an initial one-year contract and fees must be payroll deducted monthly. After the initial first year, membership fees will continue to be payroll deducted on a month-to-month basis.

Cancellations

After the initial first year, your membership can be cancelled at any time with written notice. Cancellation during the initial one-year contract period will only be permitted in cases of physician-verified illness, a move out of the 50-mile radius from NC State, or employment termination. A $30 service fee will apply to all cancellations during the initial one-year contract period. Cancellations must be submitted prior to the 8th day of each month to be removed from the next month’s deduction file. Please submit this form in-person at the Member Services Suite to process your cancellation.
